Should I drop the liner to repair rusted eye return or will install from outside work the same

Your return should be all plastic. If something is rusting, it may be the wall of your pool. Then you really do need to get behind that liner, and even then it may be be able to be repaired. Once that starts rusting, it becomes a weak point, and your pool can open up like a zipper along it (happened to my brothers pool).
