I believe I found the answer to why the tracks are coming off. The left and right side covers have a bore for the drive wheel to be inserted into it. There are 4 tangs in the bore that align the drive wheels straight when it's spinning. Once the tangs are worn out, it'll cause the wheel to spin off at an angle. The top tang seems to be one worked the most. As the motor drives the belt and load is applied onto the drive wheel creating a downward pull/load, most of the load is applied towards top tang (the one that is most worn). The opposite tang is less worn because the load is offset to the top tang under drive load. You will also notice that the drive side of the belt is the one that tends to pop off.
