Floor return unscrewing itself

This is my first season using the floor return. Last season my pool was built and builder said don't turn on while plaster is curing.

I noticed it fell off a couple days after they opened the pool. I went in and screwed it back in and noticed with water going through it was slowly unscrewing itself. I've asked the builder and they're really hard to get a hold of since we're done paying.

Does this need a screw placed? Or do I need to tighten it every few days? I turned off the floor returns because it's been too cold to swim here recently.

They are adjustable to balance the flow between them all. Notice the screw that holds the setting:

So I'm guessing I accidently vacuumed up the screw holding it on, or they didn't install it properly. Thank you!
